Gitlab - Failed to create repository

Running gitlab-7.7.2_omnibus.5.4.2 on httpd/Apache 2.2.x ProxyPassed

Am able to login, access Gitlab but when I try to create a repository I receive

• Failed to create repository

I’ve run the gitlab-shell check in /bin, all looks good. Ran the permissions line from the public-wiki on GitHub and that’s fine. I’m lost as to why no repositories will create. The logs are empty, it seems like a git/permissions issue almost.
