Gitlab Certified Associate Submit Form Does not work

Similar problem to previous Issue of No Submit Form Available , but on latest update still doesn’t work.

Upgraded from to latest version 1.18 and when clicking the final step to submit form the page shows as error

'might be temporarily down or it may have moved permanently to a new web address.

ERR_TUNNEL_CONNECTION_FAILED’
